OBJECTIVE: Previous studies on the association between migraine and the risk of developing hemorrhagic stroke (HS) have generated inconsistent results. The aim of the present population-based, age- and sex- matched follow-up study was to investigate whether migraine is associated with an increased risk of HS. METHOD: A total of 20925 persons with at least two ambulatory visits in 2001 with the principal diagnosis of migraine were enrolled in the migraine group. The non-migraine group consisted of 104625, age- and sex- matched, randomly sampled subjects without migraine. The two-year HS-free survival rates for these 2 groups were estimated using the Kaplan-Meier method. Cox proportional hazards regression was used to estimate the effect of migraine on the occurrence of HS. RESULTS: During the 2 year follow-up, 113 subjects in the migraine group (0.54%) and 255 in the non-migraine group (0.24%) developed HS. The crude hazard ratio (HR) for developing HS in the migraine group was 2.22 compared to the non-migraine group (95% confidence interval [CI]: 1.78-2.77, p<0.0001) and the adjusted HR was 2.13 (95% CI: 1.71-2.67, p<0.0001) after controlling for demographic characteristics and comorbid medical disorders. Eye movement dynamics and cognitive self-organization in typical and atypical development.

This study analyzed distributions of Euclidean displacements in gaze (i.e. “gaze steps”) to evaluate the degree of componential cognitive constraints on audio-visual speech perception tasks. Children performing these tasks exhibited distributions of gaze steps that were closest to power-law or lognormal distributions, suggesting a multiplicatively interactive, flexible, self-organizing cognitive system rather than a component-dominant stipulated cognitive structure. Younger children and children diagnosed with an autism spectrum disorder (ASD) exhibited distributions that were closer to power-law than lognormal, indicating a reduced degree of self-organized structure. The relative goodness of lognormal fit was also a significant predictor of ASD, suggesting that this type of analysis may point towards a promising diagnostic tool. These results lend further support to an interaction-dominant framework that casts cognitive processing and development in terms of self-organization instead of fixed components and show that these analytical methods are sensitive to important developmental and neuropsychological differences.

Determining the Need for In-Patient Rehabilitation Services Post-Stroke: Results from Eight Ontario Hospitals.

BACKGROUND: The number of patients requiring in-patient rehabilitation services following acute stroke is unknown. METHODS: All consecutive patients admitted with a diagnosis of stroke to eight community hospitals in southwestern Ontario from May 2008 to December 2009 were screened for in-patient rehabilitation eligibility using the Stroke Rehabilitation Candidacy Screening Tool. RESULTS: Three hundred ninety-six patients were included, of which 147 (37.1%) were identified as candidates for in-patient rehabilitation. Of these patients, 111 (75%) were discharged to an in-patient rehabilitation unit. The most frequently documented reason that candidates were not transferred was lack of an available bed (n=19). Two hundred forty-nine (62.9%) patients were not considered candidates. The majority (80%) of these patients had experienced either mildly or severely disabling stroke and went home or directly to long-term care upon discharge. CONCLUSION: The reported estimate of 37% who required in-patient rehabilitation services is important for the purposes of planning and allocation of healthcare resources.
