This is the case report of a 32-year-old obese male with a history of agitation, hallucinations, and delirium, recently diagnosed with primary hypothyroidism; he gave a several month history of fatigue with nocturnal snoring and frequent awakening. Polysomnogram revealed severe OSA; initiation of CPAP and levothyroxine resulted in immediate improvement. The lack of a previous psychiatric history and acuteness of presentation was consistent with hypothyroid psychosis complicated by sleep deprivation cause by untreated OSA. Primary hypothyroidism is a common disorder often associated with depression. It is rarely associated with psychosis and was first described as “myxoedematous madness” in 1949. It has not been previously reported to cause psychosis when associated with obstructive sleep apnea. This case illustrates the need for examination of potential multiple organic causes in a patient who presents with psychosis in the critical care setting. Subjective assessment of quality of sleep in chronic obstructive pulmonary disease patient and its relationship with associated depression.

The quality of sleep in chronic obstructive pulmonary disease (COPD) patient from India has not been studied. Aim of this study was evaluation of subjective assessment of sleep quality in stable COPD patients and its relationship with associated depression.Forty clinically stable COPD patients were recruited from outpatient department and their disease status was classified as per Global Initiative for Chronic Obstructive Lung Disease guideline. Presence of depression was assessed by administering Patient Health Questionnaire (PHQ)-9 and subjective quality of sleep was measured by Pittsburgh Sleep Quality Index (PSQI).All study subjects were male and mean age of study population was 62.2 ± 9.2 years, 12 patients (30%) in stage II, 19 patients (47.5%) in stage III and 9 patients (22.5%) in stage IV were enrolled. All subjects had poor sleep quality with the median global Pittsburgh Sleep Quality Index score 11. PHQ-9 score was significantly correlated with daytime function and global PSQI score (P<0.01). No correlation of global PSQI score with severity of COPD was observed.The prevalence of poor sleep quality among COPD patients is high. Irrespective of severity of airflow obstruction, the presence of depression in COPD is a risk factor for poor sleep quality. HubMed – depression

ABSTRACTThe growing problem of obesity is obviously quite complex and there is no one factor that can explain it. One important contributor to the obesity epidemic, however, is mental health. As we know there are bidirectional links between mental illness and weight suffering from depression, for example, may experience weight gain secondary to sleep disruption or struggle with physical inactivity that can cause weight problems, and conversely, with weight problems you may experience poor body image or physical health problems such as chronic pain that can contribute to mood problems. A focus on healthy weight, and healthy weight gain, has the potential to impact both psychiatric and neonatal outcomes through direct and indirect mechanisms and needs to become part of routine care of patients.
